What is BMAC and how is it different from PRP?

BMAC (bone marrow aspirate concentrate) is drawn from the patient’s own bone marrow, typically at the hip, and concentrated to isolate mesenchymal stem cells and growth factors. PRP is derived from blood. BMAC is generally positioned for more advanced cases or patients who need a stronger regenerative signal than PRP provides.

Does BMAC work better than PRP?

Evidence is mixed. Some studies show BMAC outperforming PRP and hyaluronic acid for pain and function, particularly in more advanced arthritis, but the overall evidence base for BMAC is smaller and less standardized than PRP’s. Preparation protocol and cell concentration significantly affect outcomes, similar to PRP.

Is the BMAC procedure painful?

The bone marrow draw involves brief discomfort during aspiration, typically from the pelvic bone (iliac crest). Light IV sedation, as used in F2 Ortho’s OR based BMAC procedures, meaningfully reduces procedural pain compared to local anesthesia alone.

How long does BMAC take to show results?

Clinical studies generally show meaningful pain and function improvement by 6 to 8 weeks, with continued improvement through 6 to 12 months in appropriately selected patients. Results are not immediate.

Is BMAC FDA approved?

BMAC is an autologous, minimally manipulated tissue procedure regulated under FDA’s same day, same surgical procedure exception (21 CFR 1271), not an FDA approved drug or device. It is a legal, regulated procedure but not “FDA approved” in the way that phrase is typically used for a drug.
