Nancy Yen Shipley, MD LLC d/b/a Form & Function Orthopaedics is an orthopaedic practice owned and operated by Nancy Yen Shipley, MD in Oregon. The Site provides general information about our services, educational health content, and a patient intake and communication portal. Use of the Site does not constitute a physician-patient relationship.
